My last name was change through the court in NYC (not marriage) and they took my birth certificate. When I went to VitalChek to order a new certificate I came across this statement:. . "Legal Name Changes. Please note that if you legally changed your name (other than for marriage) and have not AMENDED the certificate with the Corrections Unit of the New York City Department of Health, you must submit your application by mail. In addition, you must submit the original court order of the name change, a copy of your valid photo identification including signature, and the appropriate payment. All original documents will be returned to the applicant.". . When you get a court ordered name change do they notify the Corrections Unit of the New York City Department of Health? If not, how do I go about getting my new legal last name on birth certificate?. Sympathetic Stranger: When I show my court ordered name change document & ID at the NYC DOH does my ID have to show my new last name? I am not sure this will answer all of your questions, but the best thing you can do is try to relax a bit, your nervousness will be evident and not portray very well.. . Cover Page -- either put a cover page on your resume and photo sorta like an introduction letter with the resume and photo as supporting documents to your letter of introduction.. .
