I sell templates but they are just that... Templates, if you want a website ready to go then you need to get a website.

I would like to make a form where a user can submit their email address and that email can be sent to my email with "newsletter subscription" in the subject line. I figure this should be simple enough to do, but i know next to nothing about forms.

Advanced directives are to give someone in the family the right to speak for the patient because they know the patients wishes. If a patient is in surgery or under the influence of strong medications, they cannot answer for themselves. Having this form filled out and signed appointing someone is the best because then they can get direct information from the doctor, nurses, and also if they need information to help the patient apply for disability and other benefits with the appropriate medical records to back them up.

There are examples of real query letters and what did and did not work for the agent/editor. Sometimes it is very subtle, sometimes not.

ok . well. i live in the uk and leaving year 11, i applied for a sixth form in horsham west sussex called christs hospital. i had to do assessments for the subjects i wanted to take as an AS (biology, chemistry, philosophy and business) anyway, i dont even want to sound boastful but i think i did pretty well on it. but on friday 22nd i got a letter saying i didnt get it, there wasnt a reason but just that i didnt get in. now i dont want to sound rude to the people who made the decision and i dont want to come across as rude or 'in your face' but do you think i should send an email and ask the actual reason why? or should i just leave it. ok. ill call. but how should start an email as well!.
