    where can I find free name change forms for a minor?

    I have a new computer Windows XP and I cannot find how toi open up with a blank page to write a letter so I need help.

    a revocable Trust can be revoked and the Grantor can reclaim the Trust assets. An irrevocable Trust can?t be revoked once it has been set up.     Does a person merely having a blood test have a right to request that his/her sample be destroyed and expect that it be done? Fore example, whenever a clinic or hospital ( or research facility) merely does phlobotamy and sends the blood sample to a diagnostic lab, sometimes the hospital has no power or agreement with the lab about what is done with the blood sample after the results are obtained. Does the patient have a right to find out about and contact the diagnostic lab and ask for information about what will be done with the sample and request it be destroyed rather than re-used for anything else not officially authorized by the patient?.
