Goforth Co., L.P.A., formed a new partnership. Richard Goforth, located in New Philadlephia, Ohio practiced Workers’ Compensation exclusively. Tom and Rich opened their first office in Canton, Ohio and focused primarily on Workers’ Compensation but added Social Security Disability and Personal Injury practice areas. Q1: The Notary should have recorded and verified who the witnesses were. If they didn't you can re-notorize it. In California, you also have the option of doing a holographic will (100% hand written) which doesn't require any witnesses.. . Q2: It sounds like you're mixing Power of Attorney with a Living Will. If you want them to pull the plug and/or Do Not Resuscitate (DNR), that's handled via a Living Will.. . A Durable Power of Attorney (one that survives intact after you become incapacitated) is so that you designate who runs/maintains your assets while you're on life support or whatever. They're not used to establish your wishes about being unplugged or whatever.

Ever since i was a little kid I've always wanted to my husband to take my last name. Well now im grown and engaged but my husband is in the Navy, and I'm not sure he would be up to the idea anyway. However he is also aware of how attached I am to my name. So my question is can i take his last name as my middle name and continue using my maiden name? Or is it possible for him to change his middle/last name to my maiden name?. @love life -- I told him and asked him to think about it and tell me what he thought, and then we would decide.
