at birth my mother gave me her last name, while giving me my father and grand father's first and middle name. i am fourteen and i am curious about changing my last name to mach my father and grand father's full name. so my question is am i aloud to change my name with only my fathers and NOT my mothers consent, and if so what is the legal process of doing so?.

People change names all the time due to marriage, divorce, etc. Just changing your name does not invalidate the life insurance beneficiary information--in fact, that is why they require the SSN. The insurance company cannot check your name in the SSN database and will have no reason to unless your father dies. In that case, bring your SSN card, the official legal paperwork related to your name change(s) and current ID. This should not present a major problem, contrary to what some other answers have related.. . If you don't believe this, run it by a free legal clinic in your area.. . I hope this helps.

I'm planning to change my name legally using the FreeDeedPoll website, but after I've filled out the form (which I have) what do I do to verify the change? Do I just have to send it to banks etc. and if so do I have to pay for entirely new things like passports?.
