Departmen1, Question1, Does the person show up to work . Departmen1, Question2, Is the person technically competent.. Department2, Question1, Does the person show up to work . Since this may be repeating many of the questions, you could instead split it into 2 tables: Put all the questions in a table. with a number and a question. and then another table for the Question Associate to a department. with maybe three columns: The autonumber, the Department number, and the q1, q2, q200, etc that link to that question.. something like:. 1, Dept1, quest1,. 2, Dept1, quest2.. 3, Dept2, quest1,. 4,Dept2, quest12.. . Also consider if you want a table to maintain a link between the person, the department question, the date the question was evaluated, and the answer. This would be to maintain permanent records of the questionaire.. . You could would then use forms to fill out your parts, have it generate the report with the person info, the basic question info, and the department's questions with room for blanks space if needed.. . If you become good at this keep your stuff in a back-end database and create a front-end so that each supervisor can fill it out for his people in a form and have it automatically kept in the database.. . I would say, Keep the Excel tables going, but work on doing this database. I did something similar for a Complaint database, it took me about a year. I still need to work on a few things, But it is functional and can generate a report for the files.. . This would be good for probably a mid-size company, But if you are looking at a lot of people, you may want to consider SQL. In short, SQL does to Access what Access does to Excel.
