Briefly: ( and the exact procedure may vary in different Countries ) it is a legally drawn up document made during a persons life time, and registered with the Courts whereby a person nominates and declares that someone else, often a close relative or friend, will have the legal powers to act and make decisions on the makers behalf. For example, a person becomes incapable of handling their everyday domestic affairs so a relative has the power to make decisions, write out cheques, those sort of things.. . There are costs involved and in the UK the procedures were tightened a year or so back now, and the cost of drawing up a p of a increased.. . Solicitors involved in drawing up powers of attorney will need to be satisfied the maker is capable of making a valid decision to draw up a power of attorney in the first stage so it needs to be done before things get too bad.. . You mention your Father is very ill, you may want to consider his wishes with 'an advance directive'. This is a clear direction from an ill person as to what, and how far they want treatment to be made. For example some people, recognizing the dilemma between invasive treatment which may only extend their lives a short time give directions they would rather have as comfortable an end as possible and not receive drawn out treatment that may have additional unpleasant side effects eg. some chemotherapy treatments.. . Health Care is simple. Health insurance comes in various shapes and sizes, based on how much you can afford each month.. You have many different companies to choose from, who will offer their basic package, which can be 'upgraded' to include several different areas, based on your needs.. . Me, for example, have a basic package (sickness, hospitalization, medication, things out of my hands) and have, for lack of a better word, 'expansion packs' for my glasses and contacts, psychotherapy (in case I go nuts) and extensive dental expenses (in case all my teeth get knocked out by someone).. . My 4 year old son is on my insurance, so I pay a standard amount every month and don't have to worry about anything.. Normally you don't even see the paperwork. Your local hospital knows your records, so send their bill to your insurance company automatically.. . Sometimes (like in case of my glasses and contacts) they only pay up to a certain amount, the rest you have to pay for yourself, but I've hardly ever had to have to pay for anything myself.. . Well, in a sense I do pay for it.

It is a government form that should be available free. You definately should not pay for it. Look up your local court, they should have it on their website somewhere.
