File suit to change your name, and if the judge says no, because your dad didn't sign off, then amend the case to be for Emancipation and Name Change. Clearly show your existing contract number at the head of the letter, after the 'Dear Sirs' but before any text. Something like: "Notice of termination Contract ref ......... (whatever it is)". . 3.
