I think I am eligible to apply for maternity allowance. . I am currently in my probation period and I intend to leave my job before my probation period is over. I highly doubt that I am going to get maternity leave but I need my employer to complete an SMP1 form. . Im about 21 weeks and I want to leave in about 3 weeks time when I will be 24 weeks, I have accrued two weeks holiday. 29th july is when i am 15 weeks before my due date. . Am i supposed to write a letter asking them to give me maternity leave first and then they give me the SMP1form back refusing it which I then submit with my application for maternity allowance?Or do i just give them my Matb1 form and then hand in my notice ?. . Any advice or letter template greatly appreciated.. . thanks.
