  • From the time John was 8 years old, he wanted to be a lawyer.?For practically all his life, John?has lived in Cary, North Carolina. He attended the University of North Carolina in Chapel Hill and Campbell University School of Law. While at Campbell, John was named to two national moot court teams, and the law school's faculty recognized him as one of the nation's best, up-and-coming lawyers. After graduating from Campbell with honors, John initially worked for a law firm that defended claims filed by injured people. After seeing from the inside how insurance companies and big business attempt to work the legal system to maximize their own profits, John quickly realized that accident victims needed a strong and aggressive advocate to stand up and fight for them. So, in 1995, he changed firms and since then, he has focused exclusively on representing people throughout the country who have been injured by trucking collisions, medical negligence, defective products, and on-the-job accidents.

Known for his strong work ethic and attention to detail, one of John's most notable achievements occurred in 2000 when he represented a young mother and her daughter in a case against the State of North Carolina for serious injuries that they suffered when their mini-van was struck head-on by a Department of Transportation dump truck. At that time, a damages cap limited the amount of damages that could be recovered from the State of North Carolina to a maximum of $150,000, which was less than half of the mother's medical bills. Through John's efforts and against considerable odds, he was not only able to persuade the General Assembly to increase the cap to $500,000, but he also instituted ground-breaking lawsuits that resulted in the family recovering a settlement that was more than 10 times the amount initially offered by the State.

John is licensed to practice law in all North Carolina federal and state courts, and he is a very active member of the North Carolina Academy of Trial Lawyers and the American Association for Justice. John devotes a considerable amount of his time to teaching legal concepts and advocacy skills to other attorneys and law students, and his articles are frequently published throughout the State.

. . "As of June 1st 2009 I no longer wish to be known as Maureen Jane Bloggs but as Russellina Poppyhead Brand".. . You declare that to the solicitor, he signs the document and you're good to go!.

Are the shortcuts of keyboard is available in form builder?

I completed the forms and had them notarized. I then gave copies to each of my doctors, the two hospitals I use, and the persons named on the directives. I have extra copies which I use any time I see a new doctor. I think everyone should have these advance directives in order

B. Advanced directives.. . The American Bar Association, on its website, summarizes it as follows (see Source, below):. . "The PSDA [Patient Self-Determination Act] simply requires that most health care institutions (but not individual doctors) do the following:. . "1. Give you at the time of admission a written summary of:. your health care decision-making rights (Each state has developed such a summary for hospitals, nursing homes, and home health agencies to use.). the facility's policies with respect to recognizing advance directives.. . "2. Ask you if you have an advance directive, and document that fact in your medical record if you do. (It is up to you to make sure they get a copy of it).. . "3. Educate their staff and community about advance directives.. . "4. Never discriminate against patients based on whether or not they have an advance directive. Thus, it is against the law for them to require either that you have or not have an advance directive."

An allotment is a share set aside for a specific purpose. The allowance of a specific amount or of a particular thing to a particular person.

There is no "fixing" a birth certificate. It is your core legal document and record of a factual event. You were born on that date/ place and given that name. LEGAL documents should be the same as that. . If a woman gets married, the marriage record shows that event and is a legal change of name (unless she retains her birth name). It does NOT alter the original .. it is a record of a NEW EVENT. The same applies to a legal change of name. The court order will show your new name, it is then your legal identity. But it is not retroactive. You don't alter history. . If there was an error on the original, it normally must be corrected within a reasonable time, ie by the parent. . In fact, your social security card SHOULD REFLECT THE BIRTH CERTIFICATE. I dispute them telling you to fix the birth certificate. It is their error.
